JCAHO requires hospital and other healthcare organizations to perform one FMEA
a year. Krouwer Consulting has developed software
to help users fulfill this requirement. IFTF helps identify risks with process
flowcharts and fault trees. FMEA is used to classify and rank risks and allows
one to propose mitigations for the risks identified. These tools are commonly
used in engineering and are also described in the ISO document, 14971:
APPLICATION OF RISK MANAGEMENT TO MEDICAL DEVICES. A tutorial / manual is
included.
Developed in visual basic.net, the software is flexible and has among its
features:
- Create process flowcharts simply, by dragging shapes and lines - no
additional software needed.
- Fault tree and FMEA information stored in a database
- FMEA record filters allow one to show only records that meet selected
criteria
- Export FMEA reports to pdf, Word, or Excel files
- User configurable features allow conditional highlighting fault tree
events with color
- Risk Map and Risk Chart show a severity by probability display of events
- Choose between two types of Pareto sorting
- Part or all of one project can be loaded into another project
Krouwer Consulting provides training in FMEAs.
The use of fault trees while new to many people in hospitals, can actually
streamline the FMEA process.
